My Birthday with the IDF in Lebanon: A Reflection on Service and Sacrifice

As I prepare to celebrate my 58th birthday with the men and women of my PALMAR Combat Medical extraction unit in the north of Israel, I find myself reflecting on the journey that has led me to this moment. Three years ago, in the aftermath of the October 7 massacre, I made the decision to return to the IDF, becoming the oldest serving combat medic in the IDF. Little did I know that this decision would mark the beginning of an ongoing chapter of service and sacrifice.

As I am in the midst on my eleventh tour of duty, having dedicated hundreds of days to the IDF, I am reminded of the profound reasons that drive me and my fellow soldiers to return repeatedly to milluim (IDF reserve duty). These reasons speak to the core values that guide us in our service to our people and our country. From a sense of duty and honour to a commitment to defending our homeland and our beliefs, and saving lives, each reason underscores the deep-rooted convictions that fuel our unwavering dedication.

The concept of the PALMAR unit, a combat medical extraction unit in which I serve, stands as a testament to the swift and efficient response required in times of conflict. Unlike the field hospitals depicted in American TV series like M*A*S*H, our unit operates as the medical cavalry, ready to provide immediate and life-saving care to our wounded soldiers. Working in tandem with the Air Force Search and Rescue Unit 669, we stand at the forefront of medical response, ensuring that every soldier receives the critical care they need in the most expedient manner possible.

The stories of our soldiers, each one a testament to courage and sacrifice, underscore the resilience and unity that define our mission. Lt. Colonel “D” captured the essence of our work when he remarked,

When the troops see us coming, they know that they can get back to fighting because the door to the hospital has just been opened.

This sentiment encapsulates the vital role of the PALMAR unit in providing hope and relief amidst the chaos of conflict.

The success of the PALMAR unit lies in its seamless coordination of care; from the moment a soldier is wounded to their arrival at the hospital. The dedication and expertise of the medical staff, coupled with innovative technology and real-time communication systems, ensure that every soldier receives the best possible care under the most challenging circumstances.

As I look back on my years of service with the PALMAR unit, I am humbled by the opportunity to stand alongside my fellow soldiers in the noble mission of saving lives and defending our homeland. Their unwavering commitment, compassion, and pursuit of excellence make them true heroes, and it is truly an honour to serve with them.

On my birthday, as I celebrate with my comrades on the northern front, I am reminded of the sacrifices made and the lives saved by the men and women of the PALMAR unit. Their dedication and courage inspire me every day, and I am grateful for the opportunity to stand with them on the front lines, united in our shared mission of service and sacrifice.

About the Author
Tuvia Book has a doctorate in education and is the author of "Heroes of Palmar, How One IDF Unit Revolutionized Combat Medicine in Gaza" (Gefen: 2026). He is also the author and illustrator of the internationally acclaimed Israel education curriculum; "For the Sake of Zion; A Curriculum of Israel Studies" (Fifth edition, Koren), "Jewish Journeys, The Second Temple Period to the Bar Kokhba Revolt, 536 BCE-136 CE," (Koren), and “Jewish Journeys, The First Temple Period, 1000 -586 BCE” (Koren). Dr. Book is an much sough after international speaker, licensed tour guide and has been working in the field of Jewish education, both formal and informal, for many years. Tuvia has lectured throughout North America, Australia, Europe, and South Africa. Dr Book has served in reserves (Milluim) in the IDF in the current “Swords of Iron” war since October 2023 in a medical combat search and rescue unit (Palmar), and is the recipient of a prestigious IDF battalion award for his outstanding contribution to the unit.
